Here’s a more detailed answer from someone who spent a lot of time researching the science and reading real-world patient stories (including my own).

Biologics are indeed powerful medications designed to target specific components of the immune system, like TNF-alpha, interleukins (e.g., IL-17, IL-23), or T-cells, in order to calm inflammation and reduce psoriasis symptoms. But rather than “correcting” the underlying cause, most of these drugs work by suppressing immune pathways that are considered overactive.

That might sound like precision targeting, and in some ways it is, but the reality is that it still comes down to disabling, or interfering with, portions of the immune system. These drugs don’t just affect the skin; they also alter the way your body responds to infections, injuries, and even cancer cells. That’s why really every biologic drug I've ever seen advertised comes with a black-box warning and a long list of potential risks that include tuberculosis, severe infections, and even malignancies.

In essence, biologics don’t fix the root problem: they interrupt the signal. That can lead to big improvements on the outside, but it often means lifelong use to maintain results; and it may come with hidden detrimental health consequences down the road. You really need look no further than the verbal warnings accompanying the ending portion of literally every television advertisement for biologics being aired that flood TVs around the country on a daily basis to understand the inherent risks associated with these drugs.

Personally, I decided not to go that route. I spent 10 months recovering from systemic plaque psoriasis through dietary intervention, which was accomplished primarily by correcting what I came to believe was a long-standing Omega-3 deficiency. I consider myself cured of psoriasis at this point.

It’s not something I expect everyone to believe, but it radically changed my view of what “chronic” disease really is.

I’m not saying biologics don’t work. They clearly help many people. But I do believe we should all be given the full picture: not just how they work, but what they actually do, and what other options might exist beyond pharmaceutical intervention.

And just a quick clarification on one point to the first answer given here: Saying "an overactive immune system causes your body to make too many skin cells" isn’t exactly how the process works based on research I've read. It’s actually the chronic inflammation driven by immune dysfunction that creates a skin environment where cells turn over too rapidly, often 10x faster than normal.

So by that account, the immune system doesn’t directly make extra skin cells; instead it fuels an inflammatory loop that indirectly speeds up skin cell production. I only mention this because it's a subtle but important distinction that can affect how people understand both the disease and their treatment options.
