I am receiving Simponi Aria infusions every 8 weeks for my PsA. This helps but since I've been on it for 8 months, it is becoming less effective. I've tried every OTC medication and corticosteroids without any help.

That is actually what I was wondering. I will definitely check with my rheumatologist. Thank you for your reply.🙏

Could you possibly have enthesitis? I've heard people say that when they have activity it is worse feeling when they have that problem. I would ask your doctor to check for it if you don't know about it already

How can you stay physically active with psoriatic arthritis when exercise triggers flares?

It sounds incredibly frustrating that physical activity keeps triggering flares, especially when you're trying to stay healthy. The good news is there are strategies that might help.

First, it's important to talk with your rheumatologist about your medication becoming less effective. If Simponi Aria used to provide relief but isn't working as well after 8 months, you may need to switch to a different biologic or try another class of medications like JAK inhibitors or different interleukin inhibitors. Sometimes medications can lose effectiveness over time, and adjusting your treatment plan could make a significant difference.

Regarding exercise and flares, here are some approaches to consider:

Modify your exercise approach:
- Start with shorter, more frequent sessions rather than longer workouts
- Listen to your body and don't push through pain
- Work with your healthcare provider or physical therapist to identify exercises that are safe for you
- Consider gentler activities initially and gradually increase intensity

During flare periods:
- Reduce activity levels but don't stop moving entirely
- Try heat therapy before exercise to loosen muscles and increase flexibility
- Use cold therapy after activity to reduce inflammation and swelling

Prevention strategies:
- Keep a diary tracking which activities trigger flares
- Avoid injury and protect your joints from hard impacts or repetitive movements
- Manage stress through meditation, mindfulness, or other relaxation techniques

The key is finding the right balance between staying active and respecting your body's limits while ensuring your medication regimen is optimized.
