If Otezla (apremilast) is no longer providing relief for your psoriatic arthritis, several other medication classes are available. Your rheumatologist can help determine which option might work best based on your symptoms and treatment goals.
Biologic medications are commonly used when other treatments haven't been effective enough. These include:
* TNF inhibitors like Humira (adalimumab) and Enbrel (etanercept)
* Interleukin inhibitors such as Stelara (ustekinumab)
JAK inhibitors are newer oral medications that may be prescribed when biologics haven't worked well or caused side effects:
* Tofacitinib (Xeljanz)
* Upadacitinib (Rinvoq)
Note that JAK inhibitors are generally not recommended for older adults or people with heart problems.
Disease-modifying antirheumatic drugs (DMARDs) like methotrexate and sulfasalazine are another option that can help manage disease activity.
For symptom relief, your doctor might also recommend NSAIDs for pain and inflammation, or corticosteroid injections directly into affected joints for more targeted relief.
Keep in mind that some medications can take weeks or months to show results—typically around three months or five to six doses before reaching stable levels in your body.
