That sounds incredibly frustrating β€” you're doing everything right and still in pain. Hang in there. It's important to know that biologics like Tremfya can take up to 3 months to show noticeable improvement, so being only one week in means it likely hasn't kicked in yet. The pain you're feeling right now doesn't mean it isn't going to work. Since you've already tried so much, here are a few options worth discussing urgently with your rheumatologist:

- Prescription-strength NSAIDs β€” stronger than over-the-counter Motrin, available by prescription
- DMARDs like methotrexate β€” these can be used alongside a biologic to help manage inflammation while Tremfya builds up in your system
- A steroid injection directly into the thumb joint β€” this can provide fast, targeted relief lasting weeks to months (though your doctor will weigh this carefully given your skin)
- Occupational therapy β€” a therapist can suggest adaptive tools or techniques to reduce strain on your thumb during daily tasks
- A TENS unit β€” one MyPsoriasisTeam member described a portable TENS unit as "awesome for keeping the body moving when having severe pain" The most important step right now is calling your rheumatologist and being very direct about how debilitating this pain is. You deserve proper pain management while waiting for Tremfya to take effect β€” don't suffer in silence.
